DESCRIPTION:In recognition of National Healthcare Decisions Day on April 16th\, Hospice of the Upstate will be conducting a class in the two statutory forms provided by the State of S.C. for providing written instructions on life-sustaining medical procedures\; The Declaration of a Desire for a Natural Death (the so called ''living will)'' and the S.C. Healthcare Power of Attorney.  A free copy of the forms will be given to each participant.\n\n  \n\nTo make reservations for the Advance Directives class\, please call 864-224-3358 x1004\, or 800-261-8636.\n\n  \n\nClasses are free and open to the public.
