DESCRIPTION:Advanced Directives provide an opportunity to promote important discussions with our loved ones and our health care providers about the care we would want at the end of life.  Taking the burden of decision making off the shoulders of our families is a gift we can give and an expression of love.\n\n\n\nIn recognition of National Healthcare Decisions Day\, make your wishes known at our next class on Monday\, April 18th from 2pm-3pm at Hospice of the Upstate's Sadler Center (1835 Rogers Road\, Anderson 29621).\n\n \n\nFree copies of the two forms that are valid in the state of S.C. for providing written instructions on life-sustaining medical procedures\; Living Will and the Healthcare Power of Attorney will be given to each participant.\n\n  \n\nPlease call (864) 328-1966 for questions and to register.
