This role entails Case to Care Management for young fathers, serving as a liaison between the family services, school systems, court and community support entities. Case Management experience is required and a Bachelor's degree in a human/professional support services or social work is the perferred education attainment.
Vacancy Opening/Closing Date: May 16, 2019 – June 28, 2019
Case Management from: Orientation, Intake, Assessment, Record Keeping and Closure. Assess participants’ needs and determine what services are required to meet their needs. Provide assistance with daily activities such as coordinating schedules, transportation and appointment assistance, follow up and follow through.
Serve as liaison between participants, family services, courts and other contacts to assist with participants needs. Establish and strengthen relationships between Fatherhood initiative and other agencies (ie. Family Court, Child Support Enforcement, Department of Social Services, School Systems etc.)
File Maintenance, Data Collection and Reporting:
Create and maintain confidential files on participants; complete intake assessment, prepare case notes, document service requests and services provided. Maintain monthly follow up contacts and documentation.
Aptitude and Temperament:
Ability to motivate participants, present information clearly both orally and in writing, protect confidentiality of participant information, sensitivity to issues concerning diverse cultures, maintain friendly and professional working relationship with clients, family members, and the public.
Minimum Educational & Skills Requirements:
Associates degree in a related field and four years of experience in human services or social work or Bachelor’s degree in business or social discipline Experience can be substituted for education on a year by year basis. Must have a valid driver’s license and acceptable driving record, must agree to a pre-employment drug screen and criminal background check.
This is a full-time, salaried position. Salary range:$30,000 - $35,000.
