Requirements: Case Management from: Orientation, Intake, Assessment, Record Keeping and Closure. Assess participants’ needs and determine what services are required to meet their needs. Provide assistance with daily activities such as coordinating schedules, transportation and appointment assistance, follow up and follow through.
Employment Retention: Serve as liaison between participants, family services, courts and other contacts to assist with participants needs. Establish and strengthen relationships between Fatherhood initiative and other agencies (ie. Family Court, Child Support Enforcement, Department of Social Services, etc…).
File Maintenance, Data Collection and Reporting: Create and maintain confidential files on participants; complete intake assessment, prepare case notes, document service requests and services provided. Maintain monthly follow up contacts and documentation.
Aptitude and Temperament: Ability to motivate participants, present information clearly both orally and in writing, protect confidentiality of participant information, sensitivity to issues concerning diverse cultures, maintain friendly and professional working relationship with clients, family members, and the public. Requirements: Associates degree in human services, social work or a related field and four years of experience is required, or a Bachelor’s degree in business or social discipline is preferred. Experience can be substituted for education on a year by year basis. Must have a valid driver’s license and acceptable driving record, must agree to a pre-employment drug screen and criminal background check.
Salary: $32,500. This is a full-time, grant-funded position.
