Hospice of the Upstate conducts classes in Living Wills & Health Care Power of Attorneys

In recognition of National Healthcare Decisions Day on April 16th, Hospice of the Upstate will be conducting a class in the two statutory forms provided by the State of S.C. for providing written instructions on life-sustaining medical procedures; The Declaration of a Desire for a Natural Death (the so called ''living will)'' and the S.C. Healthcare Power of Attorney. A free copy of the forms will be given to each participant.
